One Umrah visa application form duly completed and signed by the applicant
Two passport-sized photographs
The Passport should be in order and valid for at least 6 months from departure date
A meningitis immunisation certificate (2 years or less children do not require this)
Children aged one year and over and travelling for Umrah on a parent’s passport must have their photograph affixed to the passport and endorsed by the authority concerned. Their photograph should also be affixed to the Umrah visa application form
All females should be accompanied by a Mahram–the husband, father, uncle, adult son, brother or nephew. The accompanying Mahram must be at least 18 years old
All non-US passport holders must be a legal resident in the US for atleast 3 months or more.
